At Affinity Home Hospice, our hospice
chaplain functions as part
of the interdisciplinary team to provide our patients and their families
spiritual and emotional support. This support takes many forms and is tailored
to fit the wishes and needs of each patient and family:
- Scheduling home visits for prayer, scripture reading, communion and other forms
of pastoral care
- Offering a confidential listening ear as a way of helping patients and
their families
cope with the stress of a difficult prognosis
- Providing our patients with encouragement and unconditional positive regard
- Working with the patient's clergy or church to supplement the patient's
existing
spiritual support system
- Assessing spiritual needs and issues
- Helping the patient in resolving any 'unfinished business' according
to their wishes
- Assisting in making healthcare decisions in accord with the patient's
values
- Helping families and patients plan for the kind of memorial
service they choose
- Conducting the memorial service in accordance with the wishes
of the patient and
family
- Providing educational resources on spirituality, grief,
and ethics
- Offering extended bereavement support for family members
in the year following
death
