What is Hospice?
Affinity Home Hospice offers holistic care for
the hospice patient and their families. A team of professionals and trained
volunteers provides the care
given to the patients and their families. It is a form of home health service
to care for people with a terminal illness who are no longer being treated
for cure. Our primary focus is on living, not dying. We believe that each
day should be celebrated and the journey one takes during the end of life
phase can be beautiful. Our staff is committed to providing the support necessary
to empower patients and their families to experience death with dignity.
When people are diagnosed with a terminal illness and have been given a limited life expectancy, they have multiple needs. Not only physical needs but emotional needs as well. Patients want to be pain free and to be free of fear. Most people want to be with the ones they love and to die with dignity. Family members also experience distress during this end of life phase for their love one. Much too often the family’s needs are overlooked and neglected. Affinity Home Hospice is committed to providing support and fulfilling the needs of both the patient and their family. Affinity Home Hospice promotes comfort, autonomy, dignity while supporting the family as well as the patient.
CRITERIA FOR ADMISSION TO HOSPICE
Physician
certifies that hospice is appropriate and patient is terminally ill with
a life expectancy of six months or less.
Patient
and family accept hospice and understand that the patient is not being
treated for cure.
Caregiver and/or the patient participate in the hospice plan of care.
