Since its inception in 2004, Affinity Home Hospice Services has demonstrated its commitment to ethical business behavior, delivering quality services, and complying with ever-changing rules, regulations and laws that govern hospices.
Affinity’s Quality Assessment Performance Improvement Committee continues to evolve with changes in the hospice industry. This Program has been developed to closely monitor both clinical and nonclinical aspects of operation to ensure optimal operation and compliance in all departments. We strive to continually improve our services and seek to change practices which strengthen our service delivery.

Affinity’s Quality Assessment Performance Improvement Committee meets quarterly to review both clinical operations data and non-clinical operations data such as financial and human resources information.
